What is a
Geriatric Care Manager?
A geriatric care manager is a professional who specializes in
assisting older people and their families to attain the highest
quality of life given their circumstances. There are many
different types of professionals who provide differing types of
services as care managers. It is important to meet with the
care manager to determine the scope of their expertise and make
sure they can provide the type of service you require.

Assessments
An
individual assessment of the client's needs should be done in
order to create a care plan-here are the steps:

Complete an individual
assessment of the client's needs

Screen the client for
depression and capacity

Make recommendations as
needed
Assessments
can be completed in either the private home or facility where
consumers reside. The person (or people) responsible for the
client must be in attendance during all assessments.
Upon completion of the assessment, a care plan with provider
listings will be provided the individual (or Power of
Attorney/Responsible Party) as indicated.
Consumer
Education, Family Support and Mediation Consults
Popular when
siblings/family members have differing ideas on what should be
done. After initial assessment and care planning, some families
(or caretaker groups) need some explanations on why a particular
care plan model may be better than others, etc. This also takes
the heat off of the individual trying to cope with being the “bad
guy” and it will lend some credibility to decisions, because a
neutral “third party” is designing the care model.
Longitudinal
Studies
These are a
system of standardized assessments that can be done quarterly to
chart functional decline over time. These are also useful in
providing caretakers with a standardized, objective tool for
decision-making. These are sent to the individual’s Primary Care
Physician (with permission) to update the Physician.
